For other insurance carriers, you will be provided with the appropriate forms for reimbursement. If you have questions about the forms, we will be happy to assist you.
It is our policy to receive payment at the time of service. Billing is expensive (anywhere from $.75 to $1.30 per bill) and can significantly contribute to the escalating costs of health care. For your convenience payment can be made by cash, check, Visa, Mastercard, or Discover. You are considered responsible for paying the total amount of the bill. For all balances over 60 days, you will be assessed a $12.00 service charge.
