BECOMING A NEW PATIENT

EFP is always accepting new patients. If you are interested in becoming a new patient please call our office at 754-3863. 

Please have the following information ready when you call:

Name (all family members)
Address
Phone numbers: home, work and cell
Date of Births
Social Security numbers
Insurance card

You can also complete and submit our “New Patient Information Sheet”.  A receptionist will call you within the next 48 hours to schedule your appointment.

It is our policy that all new patients schedule a complete physical as their first appointment.  It is imperative that we have a complete medical history.  In some cases, bloodwork could be ordered to be done before your physical appointment.  If you need to be seen immediately for an acute problem, we will do our best to fit you in.  Then you are to follow up with a physical appointment.
