If you do not carry one of the health insurances listed but choose to receive your care at Endwell Family Physicians, we will be happy to provide you with the amount or estimated amount that we will bill you for our health care services upon your written request. Please note that any amount provided in advance of service could vary due to unforeseen medical circumstances. We appreciate the trust and confidence you place in us every day to take care of you and your family.
If we do not participate with the insurance you carry, we will provide you with the appropriate forms for reimbursement. If you have questions about the forms, we will be happy to assist you.
If you do not have any insurance, we will provide you with a Good Faith Estimate of your anticipated charges before your visit.
It is our policy to receive payment at the time of service. Billing is expensive (anywhere from $.75 to $1.30 per bill) and can significantly contribute to the escalating costs of health care. For your convenience payment can be made by cash, check, Visa, Mastercard, or Discover. You are responsible for paying the total amount of the bill. For all balances over 60 days, you will be assessed a $15.00 service charge.
