EFP can treat your acute care needs. You may schedule an appointment with your primary care provider or visit our Walk In.

Examples of acute care include:

  • COVID-19
  • Colds, flu, and other respiratory sickness
  • Sunburns
  • Cuts and wounds
  • Sprains and strains
  • Ear, eye, and throat infections
  • Urinary tract infections
  • Yeast infections
  • Skin infections

Physicals

EFP encourages all patients who are eligible for an annual physical examination to complete this exam. This is your opportunity to focus on your specific health status and needs.

What can you expect at your physical? We will:

  • Measure your height and weight and check your vital signs.
    • This helps us establish your baseline and can cue us into changes in your health.
  • Review your personal health concerns
    • Remember that if you bring up new symptoms or chronic conditions, your insurance company may not cover that portion of your visit without requiring a copay or deductible. We are happy to schedule a follow up appointment if necessary.
  • Assess your social environment and how it affects your health
    • Your stress level, diet, activity level, smoking exposure, and other environmental factors all affect your health and well-being.
  • Review your medication list
    • Having an accurate list of medications (both prescription and over-the-counter) will help you avoid dangerous drug interactions.
  • Review your family health history
    • Health care conditions that your close family members have experienced are important indicators for your personal health.
  • Perform the physical exam
    • Your provider will evaluate each organ system.
  • Get tests and screenings ordered
    • Your provider may update vaccinations and order lab work, a mammogram, colon cancer screening, and other preventative care tests. We encourage you to complete these tests promptly and to follow up with your provider about the results.

Medicare Wellness

EFP believes in wellness at any age. Medicare and Medicare Advantage plans cover an Annual Wellness Visit with your primary care provider.

The Annual Wellness Visit is a time to review your wellness issues. It is not an annual physical or a sick visit. Also, it should not be confused with the Welcome to Medicare Physical (which is a one-time visit for patients new to Medicare).

What is covered during the new Annual Wellness Visit?

Your Annual Wellness Visit will include:

  • Routine measurements such as height, weight and blood pressure
  • Review of medical and family history
  • Establishing a list of current providers, suppliers, and medications
  • A personal risk assessment (including any mental health conditions)
  • A review of functional ability and level of safety
  • Detection of any cognitive impairment
  • Screening for depression
  • Establishing a schedule for Medicare’s screening and preventive services you qualify for over the next 5-10 years
  • Other advice or referral services that may help identify and treat potential health risks
  • Voluntary advanced care planning

What does the Annual Wellness Visit cost?

You pay nothing out of pocket for this visit. However, you may incur a copay or deductible expense if additional testing is required.

What do I need to bring with me to my Annual Wellness Visit?

Please bring with you your Medicare benefits card, a complete list of prescription and non-prescription medication, and also a complete list of any specialists (chiropractors, psychotherapists, acupuncturists, etc) that you are currently seeing.

What if I’m sick? Can I schedule my sick visit and Annual Wellness Visit at the same time?

It will depend on the severity of the illness and your provider’s schedule. If you are seen for both issues, you will also be charged for a sick visit and copays will apply.

How often can I have my Annual Wellness Visit?

You may have an Annual Wellness Visit once every 12 months.

Is the Annual Wellness Visit the same thing as an annual physical?

No. An annual physical is a much more extensive physical examination. Your Annual Wellness Visit should not replace your annual physical.

What is the difference between the “Welcome to Medicare Physical” and an Annual Wellness Visit?

Medicare covers a one-time Welcome to Medicare Physical within the first 12 months of coverage. Medicare also covers an annual wellness visit every 12 months in subsequent years.

What if I require further tests or screenings?

Medicare covers many screenings for people who are at high risk for certain diseases. During your Annual Wellness Visit, you and your provider will decide on the appropriate screenings.
