• More information about the characteristics of our patients is collected to provide culturally and linguistically appropriate services. We use this information about our patients and community to deliver evidence-based care.
• Clinical decision support is implemented following evidence-based guidelines for providing effective and efficient care relevant to your conditions. We will advise you of age-appropriate screenings and immunizations.
• We continue to monitor timely access to care and adapt to our patient’s needs. We are available for same day appointments and Walk In appointments for you and your family.
• Patients identified for care management receive a person-centered care plan that sets goals and considers patient preferences.
• We coordinate your care with labs, specialists and other care facilities.
• We’ve established a culture of data-drive performance improvement on clinical quality, efficiency and patient experience.
