Becoming a New Patient

EFP always welcomes new patients. If you are interested in becoming a new patient, call our office at (607) 754-3863 or use “New Patient Request” form to let us know you want to establish care with us. 

Please have the following information ready when you call:

  • Name (all family members)
  • Address
  • Phone numbers: home, work and cell
  • Dates of Birth
  • Insurance card
You can also complete the form below and a receptionist will call you within 48 hours to schedule your first appointment.
New Patient Request

We ask that all new patients to the practice schedule a complete physical exam for their first appointment. It is important that we have a complete medical history to be able to meet your health care needs. Please have your prior medical records sent to our office before your appointment. If you need to be seen immediately for an acute problem, we will do our best to accommodate you. We then request that you follow up with the complete physical exam. If you recently had a complete physical at another office or your insurance does not reimburse for a physical, please let us know so you can be scheduled appropriately. We look forward to providing your health care.
